Full Guidance For Dabigatran
If Creatinine Clearance <30 ml/min:
Not recommended to commence on Dabigatran
If Creatinine Clearance 30 - 50 ml/min, below 75 years of age, not taking verapamil, no gastritis, esophagitis, gastroesophageal reflux or increased risk of bleeding:
110mg Twice Daily or 150mg Twice Daily
If Creatinine Clearance 30 - 50 ml/min & age 80 or over or taking Verapamil:
110mg Twice Daily
If Creatinine Clearance 30 - 50 ml/min & age 75 - 80, or has gastritis, esophagitis, gastroesophageal reflux or increased risk of bleeding:
110mg Twice Daily or 150mg Twice Daily
If Creatinine Clearance >50 ml/min & age 80 or over or taking Verapamil:
110mg Twice Daily
If Creatinine Clearance >50 ml/min & age 75 - 80, or has gastritis, esophagitis, gastroesophageal reflux or increased risk of bleeding:
110mg Twice Daily or 150mg Twice Daily
